ABSTRACT

OBJECTIVE@#To observe the effect of acupotomy on the fat infiltration degree of lumbar multifidus muscle (LMM) in patients with lumbar disc herniation after percutaneous transforaminal endoscopic discectomy (PTED).@*METHODS@#A total of 104 patients with lumbar disc herniation treated with PTED were randomly divided into an observation group (52 cases, 3 cases dropped off) and a control group (52 cases, 4 cases dropped off). Patients of both groups received rehabilitation training of two weeks 48 h after PTED treatment. The observation group was treated with acupotomy (L3-L5 Jiaji [EX-B 2]) once within 24 h after PTED. In the two groups, the fat infiltration cross sectional area (CSA) of LMM was compared before and 6 months after PTED, the visual analogue scale (VAS) score and Oswestry disability index (ODI) score were observed before and 1, 6 months after PTED. The correlation between fat infiltration CSA of LMM in each segment and VAS score was analyzed.@*RESULTS@#Six months after PTED, the fat infiltration CSA of LMM in L4/L5 and the total L3-S1 segments of the observation group was lower than that before PTED (P<0.05), and the fat infiltration CSA of LMM in L4/L5 of the observation group was lower than the control group (P<0.01). One month after PTED, the ODI and VAS scores of the two groups were lower than those before PTED (P<0.01), and those in the observation group were lower than the control group (P<0.05). Six months after PTED, the ODI and VAS scores of the two groups were lower than those before PTED and 1 month after PTED (P<0.01), and those in the observation group were lower than the control group (P<0.01). There was a positive correlation between the fat infiltration CSA of LMM in the total L3-S1 segments and VAS scores in the two groups before PTED (r = 0.64, P<0.01). Six months after PTED, there was no correlation between the fat infiltration CSA of LMM in each segment and VAS scores in the two groups (P>0.05).@*CONCLUSION@#Acupotomy can improve the fat infiltration degree of LMM, pain symptoms and activities of daily living in patients with lumbar disc herniation after PTED.

ABSTRACT

BACKGROUND: At present, platelet-rich plasma has been used in bums, plastic surgery, surgery and orthopedics. However, due to the different standards for platelet-rich plasma preparation, the results of research on bums are controversial. OBJECTIVE: To systematically evaluate the effectiveness of platelet-rich plasma in the repair of burn wounds. METHODS: A computer-based online search of CNKI, VIP, WanFang, CBMdisc, PubMed, Embase, CochraneLibrary, Web of Science databases to retrieve randomized controlled trials regarding platelet-rich plasma repair of bum wounds. In the treatment group, platelet-rich plasma therapy or platelet-rich plasma therapy combined with conventional or other therapies were used. In the control group, conventional therapy, other therapies, or placebo treatment were used. After literature screening, information extraction and quality evaluation, Review Manager 5.3 software was used for meta analysis. RESULTS AND CONCLUSION: Eleven articles were included, involving 837 patients consisting of 420 patients in the treatment group and 417 patients in the control group. Meta analysis results revealed that the wound healing rate in the treatment group was significantly higher than that in the control group [OR=12.61, 95%Cl (6.93,18.29), P<0.000 1], Wound healing time in the treatment group was significantly shorter than that in the control group [OR=-4.64, 95%Cl (-6.21, -3.06), P<0.000 01]. The number of layers of gauze soaked by dressing [OR=-4.01, 95%Cl (-4.91, -3.11), P<0.000 01], the times of changing inner gauze [OR=-3.79, 95%Cl (-5.33, -2.26), P<0.000 01, and the times of dressing change on wound surface [OR=-3.41, 95%Cl (-5.93, -0.88), P=0.008] in the treatment group were significantly less than those in the control group. The positive rate of bacterial culture in wound [OR=0.46, 95%Cl (0.22, 0.94), P=0.03] and the incidence of inflammatory reaction in wound [OR=0.30, 95%Cl (0.17, 0.56), P=0.000 1] in the treatment group were significantly lower than those in the control group. The results suggested that platelet-rich plasma can increase the wound healing rate, shorten wound healing time, reduce the number of layers of gauze soaked by dressing, the times of changing inner gauze, and the times of dressing change on wound surface, and lower the positive rate of bacterial culture and the incidence of inflammatory reaction in wound.

ABSTRACT

OBJECTIVE@#To study the clinical efficacy and complications of Endobutton titanium plate and clavicle hook plate in the treatment of acromioclavicular dislocation.@*METHODS@#Total 48 patients with Rockwood Ⅲ to Ⅴ acromioclavicular joint dislocation from March 2015 to May 2019 were retrospectively divided into two groups according to different surgical methods. Among the patients, 23 patients were treated with Endobutton loop titanium plate fixation (observation group), including 15 males and 8 females, ranging in age from 23 to 59 years old, with an average of(36.2±8.1) years old;Rockwood type Ⅲ in 6 cases, type Ⅳ in 11 cases and type Ⅴ in 6 cases. Twenty-five patients were treated with clavicular hook plate(control group), including 17 males and 8 females, ranging in age from 22 to 54 years old, with an average of (34.7±6.4) years old; Rockwood type Ⅲ in 6 cases, type Ⅳ in 14 cases and type Ⅴ in 5 cases. The operation time, intraoperative bleeding, hospitalization time, visual analogue scale (VAS) of pain, Constant-Murley score of shoulder function and postoperative complications were compared between the two groups.@*RESULTS@#All the patients were followed up, and the duration ranged from 24 to 51 months, with a mean of (30.5±6.5) months. The amountof bleeding and hospitalization time in the observation group were (71.9±4.0) ml and(8.2±1.6) d respectively;and those in the control group were (97.6±13.4) ml and (12.8±1.2) d respectively. There was significant difference between the two groups (@*CONCLUSION@#Compared with clavicular hook plate internal fixation in the treatment of acromioclavicular joint dislocation, Endobutton loop titanium plate internal fixation technology has the advantages of less surgical bleeding, shorter hospitalization time, less postoperative pain, good recovery of shoulder joint function and less complications.

ABSTRACT

BACKGROUND: Soft silicone silver ion dressing, a kind of high molecular weight organic compound prepared with Safetac technology, has the characteristics of anti-infection, good permeability and good air permeability. It can provide a good moist environment for wound healing. OBJECTIVE: To evaluate the effectiveness of soft silicone silver ion dressings in repairing deep II degree and milder burn wounds. METHODS: A computer-based retrieval of CNKI, VIP, WanFang, CBM, PubMed, Embase and Web of Science databases was performed. The randomized controlled trials concerning soft silicone silver ion dressing for repairing deep II degree and milder burn wounds were selected. The treatment group was treated with a single soft silicone silver ion dressing, or a soft silicone silver ion dressing combined with conventional treatment, or combined with other treatments. The control group was treated with conventional treatment, or other treatments, or placebo. After screening, data extraction and quality evaluation, meta-analysis was performed using Review Manager 5.3 software. RESULTS AND CONCLUSION: (1) Eleven articles were included, involving 784 patients, 381 patients in the treatment group and 403 patients in the control group. (2) The results of meta-analysis showed that the burn wound healing rate in the treatment group was higher than that in the control group [OR=11.56, 95%CI (9.60, 13.53), P < 0.000 01; OR=13.29, 95%CI (7.54, 19.04), P < 0.000 01; OR=4.10, 95%CI (2.28, 5.93), P < 0.000 1; OR=1.17, 95%CI (0.42, 1.93), P=0.002] at 7, 14 and 21 days, and the last measurement. The healing time of burn wounds in the treatment group was shorter than that in the control group [OR=-2.81, 95%CI (-3.49, -2.12), P < 0.000 01]. The degree of pain in burn wounds in the treatment group was lower than that in the control group [OR=-0.38, 95%CI (-0.53, -0.24), P < 0.000 01]. There was no significant difference in the positive rate of bacterial culture on wound surface [OR=1.16, 95%CI (0.47, 2.85), P=0.74], burn wound infection [OR=1.44, 95%CI (0.47, 4.41), P=0.52], dressing frequency [OR=-8.04, 95%CI (-18.82, 2.74), P=0.14], wound treatment cost [OR=-98.26, 95%CI (-272.91, 76.40), P=0.27] and adverse reaction [OR=0.61, 95%CI (0.31, 1.20), P=0.15] between treatment and control groups. (3) These results show that the application of soft silicone silver ion dressing on deep II degree and milder burn wounds can improve wound healing rate, shorten wound healing time and alleviate pain.

ABSTRACT

Objective:To establish a model based on two-dimensional shear wave elastography (2D-SWE) for predicting post-hepatectomy liver failure (PHLF) among patients with hepatocellular carcinoma (HCC).Methods:One hundred and one consecutive patients with HCC undergoing hepatectomy from August 2018 to July 2019 were enrolled prospectively in the First Affiliated Hospital of Sun Yat-Sen University. Laboratory tests, shear wave elastography in liver parenchyma, and abdominal contrast-enhanced computed tomography (CT) or magnetic resonance imaging (MRI) were performed preoperatively. Liver functional reserve, liver stiffness (LS), and tumor-related imaging parameters were assessed. PHLF was defined according to the definition of International Study Group of Liver Surgery Recommendations (ISGLS). A predictive model was developed by logistic regression analysis and the performance thereof was evaluated by receiver operating characteristic (ROC) curve analysis and Hosmer-Lemeshow test.Results:PHLF occurred in 39 patients (38.9%). Logistic regression analysis identified that international normalized ratio ( OR=1.09, P=0.026), LS( OR=1.297, P=0.004) and the largest nodule diameter( OR=1.191, P=0.015) were independent risk factors of PHLF.The area under curve (AUC) of the model was 0.842(95% CI =0.763-0.921), which was significantly higher than those of ALBI score, MELD score and Child-Pugh score (AUC 0.626-0.688, P<0.05). The model also showed good calibration in Hosmer-Lemeshow test ( P=0.498). Conclusions:A model based on 2D-SWE provides good preoperative prediction of PHLF among patients with HCC, which might have the potential in better customizing treatment strategy in those patients.

ABSTRACT

Objective:To qualitatively analyze the elasticity characteristics of boundaries and surrounding liver tissue of focal liver lesions (FLL) by real-time shear wave elastography (SWE), and to analyze the relating influencing factors.Methods:One hundred and fifty-two patients with FLLs (152 lesions) from February 2012 to October 2013 in the First Affiliated Hospital of Sun Yat-Sen University were collected. SWE of FLLs and the surrounding liver tissue was performed and baseline clinical data were collected. The elastic boundary (clear or not clear) of FLLs and elasticity distributions of surrounding liver tissue and their influencing factors were analyzed.Results:A total of 26 benign lesions and 126 malignant lesions were included in this study. Twenty-five cases (96.2%) of benign lesions showed clear elastic boundaries and 1 case (3.8%) showed unclear elastic boundary while 82 cases (65.1%) of malignant lesions showed clear elastic boundaries and 44 cases (34.9%) showed unclear elastic boundaries. Multivariate analysis showed benign/malignant lesion was an independent influencing factor for elastic boundary ( P<0.05). Twenty cases (76.9%), 6 cases (23.1%) and 0 case of liver tissue around benign lesions showed type A, B and C elastic distribution respectively while 17 cases (13.5%), 60 cases (47.6%), and 49 cases (38.9%) of liver tissue around malignant lesions showed type A, B, and C elastic distribution. There were statistically significant differences of benign and malignant lesions, different viral hepatitis backgrounds, different gender, and different ALB, TBIL, ALT levels in elasticity distributions of surrounding liver tissue of FLLs among type A, B and C (all P<0.05) while there was no significant difference in different lesion size( P=0.036). Conclusions:The elasticity characteristics of boundaries and surrounding liver tissue of FLLs on SWE images can provide a reference for differential diagnosis of benign and malignant FLLs. The elastic characteristics of surrounding liver tissue of FLLs are influenced by the nature of lesions and the background of liver disease, but are not influenced by lesion size. Further quantitative studies of elasticity characteristics of boundaries and surrounding liver tissue are needed for the differential diagnosis of different types of FLLs.

Objective@#To study the relationships between microscopic marginal extensions and tumor types, and ultrasonic characteristics of malignant liver tumors.@*Methods@#Two-dimensional ultrasonography, contrast-enhanced ultrasonography and ultrasound-guided biopsy of hepatic tumors and surrounding hepatic tissues were performed in 78 patients with malignant liver tumor. Pathological microscopic extensions were observed after hematoxylin-eosin staining of biopsy specimens.@*Results@#The microscopic marginal extension rates of hepatocellular carcinoma (HCC), intrahepatic cholangiocarcinoma (ICC) and metastatic liver cancer (MLC) were 62.5%(30/48), 87.5%(7/8), and 91.0% (20/22), respectively. For tumors with well-defined and poorly-defined two-dimensional ultrasound boundary, microscopic marginal extension rates were 50%(14/28), and 86%(43/50). Tumor type and two-dimensional ultrasound boundary were independent predictors for microscopic extension rate (P<0.05). The median microscopic extension distances of HCC, ICC and MLC were 1.0(0, 3.0)mm, 4.0(2.3, 4.0)mm, and 2.0(1.8, 4.0)mm, respectively. The distance of microscopic extension increased with tumor size. Tumor type and tumor size were independent predictors for the distance of microscopic extension (P<0.05).@*Conclusions@#ICC, MLC, and tumors with larger diameter or poorly-defined two-dimensional ultrasound boundary have a larger distance of microscopic extension.

ABSTRACT

Objective@#To compare the results obtained from the computer-aided sperm analysis (CASA) systems of the two fully-automated commercial sperm quality analyzers, Hamilton-Thorn IVOS Ⅱ (IVOS Ⅱ) and Spanish Sperm Class Analyzer (SCA).@*METHODS@#A total of 99 semen samples were collected in the Center of Reproduction of Shenzhen Zhongshan Urology Hospital from September 2018 to October 2018 and, according to the sperm concentration, divided into groups A (50 ×10⁶/ml). IVOS Ⅱ, SCA and manual microscopy were used for the examination of each sample, followed by comparison of the sperm concentration, sperm motility and percentage of progressively motile sperm (PMS) obtained from IVOS Ⅱ and SCA.@*RESULTS@#The sperm concentrations derived from IVOS Ⅱ and SCA were significantly higher than that from manual microscopy in group A ([10.24 ± 4.60] and [10.20 ± 5.11] vs [8.45 ± 4.15] ×10⁶/ml, P 0.05) or C ([102.14 ± 45.97] and [109.48 ± 46.32] vs [104.74 ± 41.87] ×10⁶/ml, P > 0.05). Significant differences were not observed between IVOS Ⅱ and SCA in the percentage of PMS ([24.21 ± 14.62]% vs [23.92 ± 15.42]%, P > 0.05) or sperm motility ([37.48 ± 19.34]% vs [37.69 ± 16.61]%, P > 0.05) in group B, nor in group C (PMS: [30.80 ± 12.06]% vs [32.98 ± 16.10]%, P > 0.05; sperm motility: [44.50 ± 15.62]% vs [47.26 ± 17.46]%, P > 0.05). Both the percentage of PMS and sperm motility obtained from IVOS Ⅱ were remarkably lower than those derived from SCA in group A (PMS: [18.54 ± 12.96]% vs [22.90 ± 12.88]%, P < 0.05; sperm motility: [26.97 ± 14.05]% vs [34.90 ± 15.18]%, P < 0.05). IVOS Ⅱ and SCA both showed a high repeatability (CV <15%), and the former exhibited an even higher one than the latter, in detection of sperm concentration, sperm motility and the percentage of PMS.@*CONCLUSIONS@#IVOS Ⅱ and SCA both had a good consistency in the results of sperm concentration, motility and progressive motility, but showed a poor comparability with low-concentration semen samples.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinical effect of exercise rehabilitation during perioperative period on residual pain, gait and activities of daily living (ADL) in patients with lumbar spinal stenosis (LSS) after lumbar percutaneous transforaminal endoscopic discectomy(PTED).</p><p><b>METHODS</b>The clinical data of 48 patients with LSS underwent PTED from December 2015 to December 2016 were retrospectively analyzed. Patients were divided into observation group and control group according to different rehabilitation patterns, being 24 cases in each group. The patients of observation group received exercise rehabilitation and the patients of control group received conventional rehabilitation. Visual analogue scale(VAS), Oswestry Disability Index (ODI) and the ratio of supporting phase were recorded before operation and 12 days, 6 months after operation. The correlation between the ratio of supporting phase and VAS, ODI was analyzed.</p><p><b>RESULTS</b>The ratio of supporting phase of observation group was significantly higher than that of control group at 12 days after operation(<0.05). The VAS and ODI in observation group were significantly lower than that of control group at 6 months after operation(<0.01). There was no correlation between the ratio of supporting phase and ODI or VAS in two groups (>0.05).</p><p><b>CONCLUSIONS</b>Lumbar percutaneous transforaminal endoscopic discectomy combined with exercise rehabilitation during the perioperative period can release or eliminate postoperative residual pain, improve gait balance, enable activities of daily living, and has a positive effect in patients with lumbar spinal stenosis.</p>

ABSTRACT

AIM:To observe the expression of microRNA-126-5p during myocardial injury and its role in myo-cardial cell injury induced by adriamycin(also called doxorubicin, DOX).METHODS: The BALB/c mouse model of DOX-induced acute and chronic myocardial injury was established via intraperitoneal injection of DOX.HE staining was applied to observe the morphological changes of myocardial tissues.Lactate dehydrogenase(LDH)in serum was detected and PowerLab system was used to detect the influence of DOX on the changes of ±dp/dtmax.The expression of microRNA-126-5p in injured myocardial tissues and the H 9c2 cells exposed to DOX was detected by real-time PCR.Gain-and loss-of-function experiments were conducted to detect the role of microRNA-126-5p in H9c2 cells treated with DOX on LDH release and caspase-3 activation.RESULTS:In acute and chronic DOX myocardial damage models in mice,HE staining showed disarranged myocardial fibers, dissolved myofibril and inflammatory cell infiltration.Higher serum LDH level and lower ±dp/dtmaxin DOX-treated mice than those in normal mice were found.Compared with the normal mice, the expression level of microRNA-126-5p was significant increased in the myocardium with DOX-induced injury.Similarly,the expression level of microRNA-126-5p was significant increased in the H9c2 cells treated with DOX.In addition, over-expression of microRNA-126-5p decreased cell viability and promoted apoptosis,while microRNA-126-5p ablation promoted the viability and inhibited the apoptosis of H9c2 cells.CONCLUSION:The microRNA-126-5p expression is up-regulated in myocar-dial injury induced by DOX,and microRNA-126-5p inhibits cell viability and promotes apoptosis induced by DOX.

ABSTRACT

<p><b>BACKGROUND</b>Nucleolin (NCL) is the most abundant RNA-binding protein in the cell nucleolus and plays an important role in chromatin stability, ribosome assembly, ribosomal RNA maturation, ribosomal DNA transcription, nucleocytoplasmic transport, and regulation of RNA stability and translation efficiency. In addition to its anti-apoptotic properties, the underlying mechanisms associated with NCL-related roles in different cellular processes remain unclear. In this study, the effect of NCL on microRNA (miRNA) expression was evaluated by generating transgenic mice with myocardial overexpression of NCL and by analyzing microarrays of mature and precursor miRNAs from mice.</p><p><b>METHODS</b>Using microinjection of alpha-MyHc clone 26-NCL plasmids, we generated transgenic mice with myocardial overexpression of NCL firstly, and then mature and precursor miRNAs expression profiles were analyzed in NCL transgenic mice (n = 3) and wild-type (WT) mice (n = 3) by miRNA microarrays. Statistical Package for the Social Sciences version 16.0 software (SPSS, Inc., Chicago, IL, USA) was used to perform Student's t-test, and statistical significance was determined at P < 0.05.</p><p><b>RESULTS</b>Several miRNAs were found to be differentially expressed, of which 11 were upregulated and 4 were downregulated in transgenic mice with myocardial overexpression of NCL compared to those in WT mice. Several differentially expressed miRNAs were subsequently confirmed and quantified by real-time quantitative reverse transcription-polymerase chain reaction. Bioinformatics analysis was used for the prediction of miRNA targets. Furthermore, in vitro experiments showed that NCL regulated miR-21 expression following hydrogen peroxide preconditioning.</p><p><b>CONCLUSIONS</b>Myocardial-protection mechanisms exerted by NCL might be mediated by the miRNAs identified in this study.</p>

Objective To explore the clinical efficacy of proprioceptive neuromuscular facilitation (PNF) and acupuncture in the treatment of naval soldiers with stiff neck. Methods Fifty naval soldiers with neck stiffness were divided into PNF group and acupuncture group (received PNF or acupuncture treatment, respectively), each group with 25 cases. The Visual Analogue Scale (VAS) score of patients in the two groups was compared immediately and at 3 months after treatment, the degree of range of motion was compared immediately after treatment, and the recurrence rate was compared at 5 months after treatment. Results The VAS scores of patients in the two groups immediately after treatment were significantly lower than those before treatment (1.9 ± 0.5 vs 7.6 ± 1.6, 2 7 ± 0.7 vs 8.0 ± 1. 4; both P0.05). Conclusion Acupuncture has a better immediate effect than PNF in treating patients with stiff neck, but PNF is simple to operate and may reduce the recurrence rate of stiff neck, indicating that PNF can be used as an alternative in the treatment of stiff neck.

ABSTRACT

Objective To investigate the structure and biomechanical property differences in different regions of the femoral head for elderly patients with femoral neck fractures,and to study its influence on internal fixation for fracture.Methods Twenty femoral head specimens were collected from elderly patients with femoral neck fracture after joint replacement.The femoral head was divided into 3 parts (lateral,inferior and medial region) with reference to anatomical markers on surface of the femoral head.After the position and drilling direction of the ring drill were determined,a circular drill was used to obtain the cylindrical cancellous bone columns with 10 mm in diameter and 10 mm in height.The data of cancellous bone columns in different regions were analyzed by Micro-CT scanning system,including bone volume fraction (BVF),trabecular space (Tb.Sp),trabecular thickness (Tb.Th),the number of trabecular number (Tb.N),the bone surface volume ratio (bone surface/bone volume,BS/BV),structural model index (SMI).Mechanical property differences of bone tissues in different regions were calculated by micro-finite element analysis.Results Bone mass in the elderly osteoporotic femoral head decreased,and there were significant differences in bone microstructure and mechanical properties in different regions of the femoral head.Bone microstructure and mechanical properties in medial region were obviously superior to those in lateral and interior region.Conclusions The bone structure and mechanical strength in medial region of the femoral head are obvious superior to those in lateral and inferior regions.The position for internal fixation should be fully considered during treatment of osteoporotic femoral neck fracture in clinic.

ABSTRACT

The basic way of invasion and metastasis of lung cancer is that the tumor cells shed in the extracellular matrix, invade the basement membrane and the surrounding tissue, infiltrate into blood flow, and then survive and transport via the blood flow. After having been extravasated, migrated and arrested in the distant site, they finally form a metastatic lesion. Some basic mechanisms are required in these steps, such as tumor stem cells, diffusion and activity of tumor cells, escaping from apoptosis, angiogenesis and lymphangiogenesis, infiltration into blood flow, circulation and exudation, and distant metastasis proliferation. A better understanding of the mechanisms of the invasion and metastasis of lung cancer will facilitate the prevention and treatment of lung cancer.

ABSTRACT

OBJECTIVE: To explore the keypoints and methods of pharmaceutical care provided by clinical pharmacists in nutritional support for patients with severe burn. METHODS: Through summarizing the pharmaceutical care process in nutrition support therapy in large area burn patients, the contents of pharmaceutical care including the requirements for thermal energy and nutrients, choice of drugs, nutrition support pathway, complication prevention, application of other special nutrients and so on were described. RESULTS: Through pharmaceutical care, the nutritional support regimen was adjusted, which improved the safety and effectiveness of drug treatment and laid the foundation for related treatments for patients with severe burn. CONCLUSION: Nutritional support is one of the important measures in the treatment of severe burn patients. Pharmaceutical care can improve the standardization and effectiveness of nutritional support treatment. At the same time, the value of clinical pharmacists can be reflected through collaborative service with doctors and nurses.

ABSTRACT

Decreasing trend has occurred in incidence of colorectal cancer in developed countries. A shift from left to right in location of colorectal cancer has been recognized,which may be associated with colonoscopy screening,aging population,diet structure modification,increased incidence of cancer and diabe-tes. Factors including sex,race and education may also play a role to some extent. Right-sided intestinal cancers have higher proportion of poor differentiation,terminal stage and mucinous component. Complications and second primary intestinal cancer are more common in right-sided intestinal cancers. Left-sided intestinal cancers tend to be well differentiated and at relatively early stage at diagnosis. With respect to molecular mecha-nism,right-sided cancers are associated with mismatch repair system,while left-sided cancers are related to p53 mutation. Based on the differences in clinicopathology and genetics,it′s implied that left-sided and right-sided intestinal cancers may belong to two different kind of disease. It′s suggested that attentions should be paid differently according to their respective characteristics in clinical practice and trials.

ABSTRACT

<p><b>OBJECTIVE</b>To explore evolution rules of phlegm and blood stasis syndrome ( PBSS) in hyperlipidemia and atherosclerosis (AS) using NMR-based metabolic profiling and metabonomic approaches based on formulas corresponding to syndrome.</p><p><b>METHODS</b>Totally 150 SD rats were divided into the normal group, the model group, the Erchen Decoction (ED) group, the Xuefu Zhuyu Decoction (XZD) group, the Lipitor group, 30 in each group. The hyperlipidemia and AS rat model was duplicated by suturing carotid artery, injecting vitamin D3, and feeding with high fat diet. ED and XZD were used as drug probes. Blood samples were withdrawn at week 2, 4, and 8 after modeling. Blood lipids, blood rheology, histopathology and metabolomics were detected and analyzed. Results Results of blood lipids and pathology showed hyperlipidemia and early AS rat models were successfully established. At week 2 after modeling, levels of total cholesterol (TC) and low-density lipoprotein cholesterol (LDL-C) significantly increased, which reached the peak at week 4 and maintained at higher levels at week 8. ED exerted obvious effect in improving TC and LDL-C levels of early models, while XZD could greatly improve levels of TC and LDL-C of late models. Rheological results showed at week 2, there was no significant difference in whole blood viscosity, plasma viscosity, or hematocrit between the model group and the normal group (P > 0.05). At week 4 partial hemorheological indicators (such as plasma viscosity) were abnormal. Till week 8 whole blood viscosity, plasma viscosity, and hematocrit were significantly abnormal (P <0. 05, P < 0.01). As time went by, whole blood viscosity, plasma viscosity, and hematocrit showed gradual increasing tendency in the ED group, while they showed gradual decreasing tendency in the XZD group. Results of metabonomics showed significant difference in spectra of metabolites between the normal group and the model group. As modeling time was prolonged, contents of acetyl glucoprotein and glucose in the model group increased in late stage, which was in. line with results of blood lipids and hemorheology. ED showed more obvious effect in early and mid-term modeling (at week 2 and 4), and increased contents of partial metabolites (such as choline, phosphatidyl choline, glycerophosphocholine), but these changes in the XZD group were consistent with those of the model group. In late modeling (at week 8) XZD showed more obvious effect in improving contents of lactic acid, acetyl glycoprotein, LDL, creatine, choline, and glucose.</p><p><b>CONCLUSIONS</b>ED and XZD not only showed regulatory effects on lipid disorders, but also could improve dysbolism of Chos. In formulas corresponding to syndrome, damp-phlegm was main pathogenesis of hyperlipidema and AS in early and mid stages. Blood stasis syndrome began to occur along with it progressed. Phlegm can result in blood stasis and intermingles with stasis. Phlegm turbidity runs through the whole process.</p>
